Most patients experience meaningful pain relief from a cortisone injection within 3–7 days. Relief that lasts 3–6 months is a typical realistic expectation — not permanent cure. If a well-placed injection provides no relief after 2 weeks, it provides important diagnostic information: inflammation may not be the primary pain driver, and the diagnosis should be reconsidered.
